Travel day is Tuesday June 4th and we will return home on Friday the June 14th.

The cost for participation in this project will be $120.00 per person. It will be due by May, 10th. (just make check payable to: Carpenters for CHRIST and mail them to: Cherry St. Baptist Church, 520 Cleveland Ave. Attalla, AL. 35954 attn. Dave Tidwell.)

Each person is responsible for any personal medication that he might need. We are allowed only to supply first aid supplies.

Each person needs to have his own medical/hospital insurance. Please be sure to have your identification card with you.

Be sure to fill out a personal information and registration form (click here for printable form) for each participant and return it with your check.

Remember, we must have a signed information form for each participant by no later than May, 10th
